INTAKE FORMS

When you schedule your initial 90-minute appointment, you will receive a link to the Client Intake Package including the HIPAA Privacy Policy and Agreements document. This package needs to be completed online before your appointment can be confirmed. The information you provide will be transmitted via a secure, HIPAA-compliant portal and will allow me to set up your account, review your needs and prepare for our session.

PAYMENT OPTIONS

Payment can be made with Visa, Master, Discover, American Express, Health Savings Account (HSA) Card or PayPal, and tax receipts are emailed to clients upon payment.

FEES

Consultation fees are USD $180 per standard therapy hour, payable at the time of service. The initial 90-minute session fee is USD $270.

CANCELLATION POLICY

This work requires mutual commitment. Please be advised that a 24-hour cancellation policy applies for all appointments. No-shows and/or last-minute cancellations will be charged for the missed session and the proceeds donated to charity.
